ELCN Advisory Board

The Elder Life Care Network™ advisory board is a seamless part of our operations. These professionals have expert knowledge in elder care and aging systems and help guide the ELCN as it expands its operations. The board has input on a consistent basis to discuss external environments, markets and industry trends as well as review business performance and define strategy.

The advisory board consists of professionals across the spectrum of elder care management including medical, case management, quality/regulation, legal and community living. Together the Elder Life Care Network executive team, and the ELCN advisory board, bring a sense of hope and possibility to elders and provide a structured path to health and elder care industry professionals.

Grace Dotson, RN, MS, CMAC

Grace Dotson is a Registered Nurse who serves as the Director of Care Coordination for the Southcoast Hospitals Group (Fall River, MA). Prior to joining Southcoast, she served as the Director of Case Management for a large academic health system in South Carolina (2008-2013) and Albany Medical Center in upstate New York (1999-2008).

Her professional expertise includes 15 years of progressive Case Management leadership within the setting of large tertiary care health systems and active engagement with the board of the American Case Management Association. Mrs. Dotson is Board certified by the Center for Case Management as a Case Management Administrator (CMAC).

She has presented for national, state and local conferences on Case Management related topics such; patient flow, the value of teamwork, use of data to set goals and monitor performance, value based purchasing in healthcare, deciphering the CMS regulatory requirements for hospital operations including; level of care determinations, readmission reduction strategies, compliance with CMS conditions of participation and the recent IPPS Final Rule/ 2 midnight rule from the perspective of the physician and case manager.

Clinical nursing experience includes; general medical & surgical acute care, neurology, acute rehabilitation (IRF), nephrology (hemodialysis, peritoneal dialysis and transplant), homecare services and patient education for post acute infusion therapies.

Mrs. Dotson promotes the use of innovative rounding among the healthcare team as a method of enhancing patient safety through improved communication among the team and with the engagement of the patient and family.

Kathleen, Calandra, RN, BSN, CPHQ

Kathy Calandra leads the Care Coordination work for Healthcentric Advisors in Rhode Island. She joined Healthcentric Advisors in December of 2011. Through her current role, Ms. Calandra oversees the CMS contracted Coordination of Care project in Rhode Island. She convenes community coalitions that bring together leaders from all care settings, to address the problems that occur during care transitions, and which cause unnecessary readmissions due to failed discharge plans. She coordinates learning activities, and presentations related to this work.

In her initial role with Healthcentric Advisors, she leads a team responsible for the review of Medicare beneficiary clinical appeals and quality concerns. She provided technical assistance and education to hospitals, and other care settings, to ensure patients were able to receive the best evidence based care in the most appropriate setting.

Ms. Calandra has a Bachelor’s of Science Degree in Nursing from Florida State University, and certification as Certified Professional Healthcare Quality. Her nursing career spanned leadership positions in hospitals, ambulatory care, and managed care organizations, and now quality improvement consulting. This path has allowed her to touch patients who were acutely ill, the elderly, and the vulnerable populations, and to apply her experience to improve the care of these patients.

Mathew Muratore

Mathew Muratore, State Representative for the First Plymouth District, was first elected to the House in November 2014. He was a Plymouth Selectman from May 2010-January 2015 and served as Chairman of the Board of Selectman from 2012-2014.

In the 2015-2016 legislative session, Rep. Muratore serves as the assistant ranking minority member of the Joint Committee on Health Care Financing; ranking minority member of the Joint Committee on Tourism, Arts and Cultural Development; and a member of the Joint Committee on State Administration and Regulatory Oversight.

Prior to his election as State Representative, Matt was a licensed nursing home administrator since 1991 and most recently the Administrator at Plymouth Rehabilitation and Health Care Center from 2003 to January 2015. Matt is also a member of the Massachusetts Senior Care Association and operates his own health care consulting company, MAT HealthCare Consultants LLC.

Dr. Deborah Kylander, MD

Dr. Kylander operates a Geriatric medical practice and Complex Care Management Program at the Plymouth Medical Group (PMG) caring for the frail elderly and patients with complex medical conditions. Dr. Kylander leads an interdisciplinary team that follows patients in the home, assisted living and nursing home settings to provide care in the areas of disease management, early identification of complications and maximization of function and quality of life.

Dr. Kylander is board certified in family medicine, geriatric medicine and hospice and palliative care medicine. Dr. Kylander has received multiple awards including the Patient’s Choice Award (2008-2011, 2013, 2014) and Compassionate Doctor Recognition Award (2011, 2013, 2014).

Adam has spent nearly 30 years in customer centric sales positions and catered his real estate firm, Milestones Realty, to the needs of seniors and their families working through the most difficult of home sales. Drawing from his own family experiences in downsizing during a strenuous transition, Adam brings an “insider’s empathy” for clients and their families when faced with similar moves. After earning his Seniors Real Estate Specialist (SRES®) designation in 2007, his passion of being an elder-friendly real estate brokerage became crystalized. Milestones Realty provides representation to this vulnerable population and currently serves the South Shore and Metro-West.

Adam serves as Vice President of the Senior Service Network of Boston South and is an active member of Plymouth & South Shore, Massachusetts, and National Associations of Realtors.

Paul R. Tierney is an attorney concentrating in Estate Planning, Asset Protection, Elder Law, Wills and Trusts, and Probate of Estates. He is a graduate of Boston Latin School, Stonehill College and Suffolk University Law School. He is a member of the following organizations: National Academy of Elder Law Attorneys, Massachusetts Bar Association, Norfolk County Bar Association, Norfolk and Plymouth Estate and Business Planning Council, and the Neighborhood Club of Quincy.
