Unmanaged chronic illnesses can lead to more intense symptoms which can progress to loss of physical and cognitive function. This vicious cycle may cause multiple problems such as ongoing fatigue and stress, falls, and medication errors.

Meet Guided Living, Our Home Care Division

Guided Living Senior Home Care is the home care service of the ELCN where the systems and reporting structures that support the ELCN are embedded into the day to day work of Guided Living Senior Home Care. The ELCN has set an extremely high standard for caregivers and home care practices. Ongoing information exchange with each individual caregiver as well as the RNCCM and home care RN staff are an integrated and crucial part of the reporting process.
