Kathleen Heery, MS, RN, CCM, CCPArchitect and Founder
Kathleen Heery has more than 30 years of professional experience holding multiple clinical and management positions and ran a private practice as an RN Geriatric Care Manager and Organizational Consultant for the past 12 years.
In the past, she has served in leadership roles as a National Director of Clinical Competence for a Large Homecare Company, Director of Performance Improvement for a National Care Coordination Organization and Global Director for Care Management Services for an international insurance organization. Kathy is actively involved with promoting positive change within the health care industry through proven project management and quality improvement approaches to community-based care delivery. She has had success in uniting cross continuum providers, and in building innovative methods that generate solutions to the challenges of aging.
Kathy is an adjunct faculty member at Fisher College teaching Gerontology and Public Speaking classes.
Lynne Furze RN, CMCRN Geriatric Care Manager
Lynne is a registered nurse and certified Geriatric Care Manager servicing the South Shore area. She served as the Director of Care Management at the Village at Duxbury, an independent and assisted living senior community, for the past seventeen years, and is now devoting her time as an RN Community Case Manager including various Geriatric Care Management (GCM) functions.
Lynne’s professional background includes consulting for the assisted living industry, offering training programs for Mass AFLA , and her previous clinical experience includes home care and critical care nursing.
Linda D. Wise RN, BSRN Geriatric Care Manager
Linda brings more than 40 years of nursing experience including decades working with geriatric populations. Her background includes various leadership and consulting roles across the healthcare spectrum as well as a clinical instructor role in a local nursing program. Additionally, Linda has spoken at Harvard Medical School Center on issues related to Palliative Care.
Some of Linda’s leadership and solution-driven approaches result from her military background as a Captain in the Army Nurse Corp–a role that instilled a focus and flexibility in the process of exploring and evaluating care options. Linda mixes this experience with her calm demeanor, patience, passion and commitment to helping elders to age well.
As a RN Community Case Manager and Geriatric care manager, Linda evaluates issues, concerns, resources to provide objective decision support and individualized plans that enable clients to live at their highest level of functioning with independence and dignity.
