Kathleen Heery has more than 30 years of professional experience holding multiple clinical and management positions and ran a private practice as an RN Geriatric Care Manager and Organizational Consultant for the past 12 years.

In the past, she has served in leadership roles as a National Director of Clinical Competence for a Large Homecare Company, Director of Performance Improvement for a National Care Coordination Organization and Global Director for Care Management Services for an international insurance organization. Kathy is actively involved with promoting positive change within the health care industry through proven project management and quality improvement approaches to community-based care delivery. She has had success in uniting cross continuum providers, and in building innovative methods that generate solutions to the challenges of aging.

As a professional speaker Kathy lectures locally and regionally on a variety of topics that include improving care transitions, understanding the basics of healthcare reform and developing new care delivery models. Kathy has published multiple books with the latest ones available in print and online, Too High a Price to Pay: The Healthcare Reform Revolution, what it is and what you can do (2014); and Managing Complex Conditions in Home Health (2016).

Kathy is an adjunct faculty member at Fisher College teaching Gerontology and Public Speaking classes.

Patrick Flaherty has spent his career fostering an entrepreneurial spirit in environments ranging from the board room of a large corporation to the ground floor of a tech start-up. A common thread throughout has been his passion for finding innovative ways to solve complex customer problems using technology.

After holding national product management roles for companies delivering both hardware and software technology products, Patrick found his great passion through a personal experience that led to the founding of Guided Living Senior Home Care in 2014. His home care company was built upon a client centric model where transparency, communication, care management, and service excellence are the top priorities delivered through a cutting edge technology platform.

Through working closely with certified and professional RN GCM Kathy Heery in building Guided Living from the ground up, a shared vision of a truly ground breaking care management model emerged. Rooted in the four pillars of care management, home care excellence, trusted partner relationships, and integrated technology the Elder Life Care Network™ was born with a mission to bring clarity to elders and their families with complex care situations.

Patrick leads the development of the proprietary ELCN technology platform and works closely with both clients and industry partners to adapt to our continuously changing health care and elder care systems.
